Exam Services
Comprehensive Eye Exam
A full eye exam determining prescription for and overall health of eyes; considered routine eye exam. Dilation is included although patient may elect to defer for another day. Dilation provides additional health information. After effects cause blurred vision for 4-6 hours. Driving and computer use is not recommended after dilation. This exam provides a prescription for glasses and full report on ocular health. Vision insurances and Medical insurances may be used for this exam depending on your eligibility.

Medical Eye Exam

Intermediate Eye Exam
A partial exam that only provides a prescription for glasses. No health checks or contact lens prescriptions are offered during this exam. Vision insurance may NOT be applied to this exam.
Exam Fee: $50
Contact Lens Fitting Exam
Considered an add-on to your Comprehensive Eye Exam, fittings are required by law to evaluate the health of the eye with contact lenses. Existing patients and those who need a new fitting must book this add-on if they wish to wear for the first time OR continue to wear contact lenses. This prescription will expire on a yearly basis and requires yearly renewal even if the exact brand and prescription is given. The exam provides a prescription for contact lenses. Vision insurance may be applied to this exam depending on your eligibility.
Non-Insured Fitting Fee: $75-$150 (prices vary due to style of contact lens)
Follow Up Eye Exam
This visit is included within the first 30 days of your original visit to address any additional concerns pertaining to your comprehensive exam and contact lens fitting. Patients who need adjustments on glasses may book a follow up exam to allow for questions and adjustments. There are no fees for follow up appointments within the first 30 days of your original exam date.
DMV Eye Exam
An exam to measure vision as determined by your state to renew your drivers license; forms to be provided by the patient. This exam will NOT provide a prescription for glasses or contact lenses. Vision insurance may NOT be applied to this exam.

Office Policies
No Refunds: Only Store Credit
Frame Warranty
Within 30 days of notification of product arrival: will replace if manufacturer defect (discoloration, absence of material, warping).
Within first year of order date: lost or broken item. Patient responsible for 50% of retail price to replace.
Lenses Redo: Only for Rx Change or Material Change
Within 30 days of notification of product arrival: no refunds, only store credit, one redo per order (lenses are a custom order).
Lens Warranty
Within first year of order date: only with purchase of anti-glare coating, one replacement within first year.
Contact Lenses Return/Exchange
Within 30 days of notification of product arrival: no refunds, only store credit. Box must be in original condition including packaging for credit or exchange (closed boxes/no markings).
Within 30 days - 6 months of notification of product arrival: $15 restocking fee for store credit or exchange product. Box must be in original condition including packaging for credit or exchange (closed boxes/no markings).
6 months - 1 year: $15 restocking fee and new exam required for new prescription. Box must be in original condition including packaging for credit or exchange (closed boxes/no markings).
Prescriptions Rx Check
1 - 30 days of notification of product arrival: no fee
30 days - 3 months of notification of product arrival: $25 fee
3 - 6 months of notification of product arrival: $50 fee
6 months - 1 year of notification of product arrival: complete eye exam required
